This Public Health Nursing position is responsible for nursing work that promotes and protects the health of individuals, families and populations using knowledge from nursing, social and public health sciences.
Work includes communicable disease investigation and case management, maternal and child health home visiting, community health education, and clinical based work, including but not limited to immunizations, sexually transmitted disease screening, tuberculosis case management, and chronic disease screenings.
The Public Health Nurse will facilitate client access to health care systems, including referrals to community resources and support.
They will also lead, participate, or facilitate the development of various community-based partnerships that promote the health of the community and incorporate the use of systems thinking to promote population health.
This position requires a self-motivated professional who provides exceptional client centered care, modeling a high level of emotional intelligence and compassion while being detail oriented.
This individual will excel at developing relationships and trust with clients and is able to effectively engage with both internal and external partners within the public health system.

Essential & General Job Functions Essential Job Functions Performs surveillance, prevention, investigation, case management, medications administration, and control of communicable disease, acute and chronic disease.
Provides community screening, health promotion, and immunization services.
Train for and participate in emergency response planning for the county, and respond in the event of an emergency.
Provide direct nursing assessment and follow-up for families and individuals regarding health care needs utilizing the nursing process.
Provide skilled nursing care and education to selected populations including Women Infants & Children (WIC), Prenatal Care Coordination, first time parents, First Breath, Lead screening, & Comprehensive Support Team Program (CST) families.
Assist with documentation and record keeping of services provided including Secure Public Health Electronic Record Environment (SPHERE), Real-Time Online Statewide Information Environment Security Awareness Training (ROSIE), Wisconsin Immunization Registry (WIR), and Wisconsin Electronic Disease Surveillance System (WEDSS).
Collaborates with public and private organizations to assess, and address community health needs.
Often taking a leadership role in group efforts.
Plans, conducts, and evaluates public health education programs for agencies, community organizations, or the general public.
General Job Functions Serves as resource person to new and/or less experienced nursing personnel.
Participates in workshops and in-service education.
Assist in orientation of new nursing personnel and student nurses.
Training, Experience, Knowledge, Skills & Abilities Required Training and Experience Bachelor of Science Degree from an AACN/CCNE or N.

accredited college or university school of nursing and current licensure as a registered nurse in the State of Wisconsin.
Valid driver’s license.
Valid American Heart Association BLS certification.
Training and experience in public health emergency preparedness, including Incident Command Structure (ICS) and National Incident Management System (NIMS) is preferred.
Must obtain IS-700 NIMS and ICS-100 and 200 Certification within the first year of hire.
Preferred Training and Experience Two or more years of clinical and public health nurse work experience.
Knowledge, Skills, and Abilities Required Knowledge of the principles and practices of public health nursing, and the sociological aspects of community health programs and of public health education programs.
Ability to exercise good independent judgment in making decisions and use of discretion in handling confidential information.
Ability to perform quality nursing services according to professional standards.
Ability to read, comprehend, and communicate, both verbally and in writing.
Ability to use tact and courtesy in maintaining an effective working relationship with department employees, county supervisors, county officials, and general public.
Ability to implement assigned components of Public Health programs.
Ability to travel to client’s home, clinics, and sites throughout the County.
Working knowledge of Microsoft® Office Suite of software and data base programs.
