We are a home health and hospice agency providing services in clients' homes from Green Bay to Kenosha and from Waukesha to Lake Michigan.
Our home office is in Germantown, with branch offices in Fond du Lac, Appleton, and Waukesha.
Our clients reside in assisted living facilities, their private home, or wherever home may be.
We bring our services to our clients.
To best provide for our clients and case managers, we are looking to add to our 7 on/7 off team.
We are looking for an RN or LPN to work Monday through Thursday 4 PM to 7:30 AM and from 4 PM Friday to 7 AM Monday.
During this time, you will have both home health and hospice visits.
Some of the visits will be scheduled visits and some will be on-call.
We have an opening for an RN or LPN in our far north territory area-Green Bay to Appleton to Fond du Lac.
SIGN ON BONUS UP TO $7,000
* This is a full-time position
* Hours are Monday through Thursday from 4:00 PM to 7:30 AM and Friday from 4:00 PM to Monday at 7:30 AM on alternate weeks
* You will have some regular scheduled nursing visits
* You will triage after hour calls and respond in person as needed

Successful applicants will possess the following:
* License in the State of Wisconsin
* Enjoy working with the geriatric population
